Form 1, “Request for Exemption from Public Disclosure of Discharge Papers” Use this form if your discharge was recorded in Pierce County between January 1, 1969 and June 12, 2002. Completing and recording this form assures that your DD214 will be removed from public access and copying. Per RCW 42.17.310, this form provides information on who may view and copy the record once it has been removed from public access. This would include the veteran and the veteran’s next of kin. Please read the document to obtain full details. Complete the form as follows; 1. The form requires the PRINTED name and the SIGNATURE of the veteran. 2. The form requires the 7 to 12 digit Auditor’s Recording Number. This number will appear at the top or the side of the original document. If you are unable to locate the recording number and the DD214 was recorded between 1984 to the present, you can use the link on the Recording Home page to Search for a Document. On-line searches require the name of the veteran. The search result will bring up the recording number of the Separation Record. Enter this number on the “Request for Exemption” form. If the discharge was recorded between 1969 and 1983 and is not on-line, the Public Access Area in our office can be used for searching records. 3. Fill out the “Designee” area on the form only if you wish to add a person other than your next of kin to access your exempt record. 4. The recording fee is $7.00 payable to the Pierce County Auditor. 5. Because this document requires original signatures, you must present it in person or mail to the Pierce County Auditor’s Office. Form 2, “Request for Disclosure of Discharge Papers” The law provides that certain DD214’s are automatically exempt from public access and that only the veteran or the veteran’s next of kin are allowed to view and copy the record. If the veteran wants to designate someone other that the next of kin to have access to the record, this form must be completed. Use this form for DD214’s recorded from 1921 through 12/30/68 AND from 6/13/2002 to date. 1. The form requires the veteran’s PRINTED name and SIGNATURE. 2. This form requires the recording number from the original DD214. This 7 to 12 digit number will appear on the top or side of the DD214. 3. Fill in the designee. 4. There is no fee to record this document. 5. Because this document requires original signatures, you must present it in person or mail to the Pierce County Auditor’s Office. Form 3, “Revocation and Re-designation of Disclosure of Discharge Papers” If the veteran wishes to change a designee or add a designee to a previously recorded “Request for Exemption from Public Disclosure of Discharge Papers’ or a “Request for Disclosure of Discharge Papers”, this form must be completed. 1. The SIGNATURE of the veteran is required. 2. The form requires the recording number of the “Request for Exemption…..” or the “Request for Disclosure of Discharge Papers”. This is a 12 digit number at the top of the document that being changed. 3. The form requires the name of the designee. 4. There is no fee to record the document. 5. Because this document requires original signatures, you must present it in person or mail to the Pierce County Auditor’s Office, 2401 South 35th Street, Tacoma, WA 98409. Form 4, “Request for Access/Copy of Exempt Discharge Paper” Anyone, other than the veteran, wishing to access or copy an exempt discharge record, must first complete and record this form. An exempt discharge is one that was recorded in Pierce County between 1921 and 1968, a discharge recorded June 13, 2002 or later or any discharge where the veteran has recorded a “Request for Exemption of Disclosure of Discharge Papers”. The veteran can access and obtain certified copies of his or her discharge and does NOT have to complete this form. ID from the veteran is required in the form of a driver’s license or a state ID card. If requesting by mail, please provide a photocopy of your driver’s license or state ID card. 1. The PRINTED name of the veteran is required. 2. The SIGNATURE of the next of kin or the designee is required. 3. The relationship to the veteran if the next of kin is accessing the record is required. 4. The recording number of the original DD214 is required. The recording number is a 7 to 12 digit number at the top or side of the discharge. 5. There is no fee to record this form. 6. There will be no fee to obtain up to 3 certified copies of the discharge. 7.
